$300 for a one-year agreement for dental services described by the following and provided by Enhance Family Dental.

• Services included with the $300 fee
  • A total of two cleanings or periodontal maintenance appointments per year.
  • Full set of X-rays every 5 years
  • Panoramic X-ray every 5 years
  • Initial exam
  • One emergency exam per year
  • Two periodic “check-up” exams per year
  • Bitewing X-rays (check up X-rays) every year
  • Periodontal probing
